Friday,
Dec. 2nd was the due date for all children who lacked required immunizations
when you completed your original State Immunization Report earlier
this year. This important State report must be returned even if
it's late. Remember, this Follow-Up Report is only mandatory for
facilities who did not have 100% compliance on the Sept. 30th Immunization
Report.
To determine if you need to complete a Follow-Up Report, check your
original Immunization Report, find box 3a.”Number of Children
with Follow-up Needed”. The number in that box will determine
how many, if any, children need follow-up. Note that you
are only responsible for following-up on the children who were in
your care when the original report was submitted. Any new
enrollees since you filed your original report need not be counted
on this follow-up. Refer to your original report worksheet for more
information.
The State requires
this Follow-Up Report to make sure that those children missing shots
get the immunizations they need and are not forgotten about until
next Fall’s State Immunization Reports. Simply put, children
don’t have all the protection that immunizations provide unless
they have all the recommended shots in the series. The State requires
this follow-up so that the children are as protected as they can
be from these vaccine preventable diseases.
It’s very
easy to download the form below, complete it, and return it either
by fax or mail. The State cannot close your reporting file until
your Follow-Up report is received, so don’t delay, fax it
in today!
Steps for completing
your Follow-Up Report:
- Find
and review your original Immunization Report.
Please note that you are only responsible for following-up
on the children who were in your care when the original report
was submitted. Refer to your immunization worksheet for
a more accurate listing of the children who were included on your
original Immunization Report.
- Pull
the Blue Cards of those children needing follow-up.
Check their Blue Cards for evidence that those children have in
fact received the immunizations they were missing. For those children
who have received all the missing shots, they are now up to date
and need no more follow-up.
- If
you find no evidence of missing shots, contact the parent(s) to
ask them to send in the child’s California Immunization
Record (the Yellow Card) asap.
Review the records when they do come in, and log dates of new
shots on the child’s Blue Card.
- If
the Immunization Record shows no new shots, notify the parent(s)
immediately that more shots are required now!
Beware: some parents may state that their child has an appointment
in X weeks or months to get the missing shots. Unfortunately,
having an appointment for shots in the future is not acceptable
proof that children will actually get those missing shots.
The missing immunizations are needed asap and proof in the form
of a shot record is required. Some doctor’s offices may
not be able to book shot appointments immediately, so the list
of low and no-cost walk in clinics may prove helpful. Click

- Now
you are ready to begin the Follow-Up Report.
- Completely
fill-out the Follow-Up Report.

personal belief or medical exemption on file), you are NOT
required to send in the Follow-Up Report.
