  HOW TO DOCUMENT IMMUNIZATIONS GIVEN ELSEWHERE  
Document immunizations given by other providers from medical documents even if the vaccine manufacturer and lot number are unknown.
  1. Verify dates of vaccinations not given in your office by:
    • obtaining prior medical records
    • taking dates from patients' appropriately documented California Immunization Record
    • looking up the patient in the San Diego County Immunization Registry
  2. Document the dates of shots given elsewhere on the Immunization Record and History Form. The full date (month/day/year) should be recorded.
  3. Write or stamp "Transcribed by" and the name of your provider and/or medical office in the manufacturer and lot number space.

Such documentation will allow you to differentiate between doses given by you and other providers.
