It’s that time of year again! Annual Immunization Reports for Childcare Centers and Kindergartens will be mailed to facilities by August 20th. If you do not receive a packet from the San Diego Immunization Branch or California Department of Public Health by September 1st, please contact Kimberly Ralston at (619) 692-8601.
Kindergartens - It’s time to report online!!
Schools will be submitting their reports through an online database this year! For instructions and access to the online database, go to www.CalKidShots.org. You will find instructions for completing the report and submitting it online.

Most of the forms needed to complete your report are here. You will need to call SDIB for a replacement “Annual Immunization Report” (CDPH 8018).

Annual Immunization Report Help Sessions
If you need assistance completing your Annual Immunization Report you are invited to attend a Help Session that will be offered for school and childcare staff. Please bring the report forms and completed worksheets for your facility. All sessions will be held in the Harbor Room at the Health Services Complex at 3851 Rosecrans Street, San Diego 92110.
